**Welcome to Providence Oregon Community Health**
Caring for the communities we serve lies at the heart of who we are. As part of our mission, it is our responsibility to care for members of our community who seek medical services as well as addressing broader health priorities.
Providence Community Health works to impact those needs identified by our most recent Community Health Needs Assessment (CHNA) and actions defined in our Community Health Improvement Plans (CHIPs). To improve the health of our communities, we foster deep partnerships with community-based organizations who share our passion and goals.
Providence Oregon service areas include Clackamas, Clatsop, Hood River, Jackson, Multnomah, Washington, and Yamhill counties.
